What insurance do you take?
Summit Psychological Assessments and Services accepts Blue Cross Blue Shield and Private Pay options. Families are encouraged to contact their insurance provider prior to appointments for a cost of service estimate. For evaluations, procedure codes billed typically include 90791, 96136, 96137, 96130, 96131, 96132, and 96133. For therapy, procedure codes billed typically include 90791 and 90837. It is the client’s responsibility to check with their insurance company before services are completed to determine coverage provided. We urge families to ask any questions they have regarding billing/fees prior to services being provided.
How long does an evaluation take?
Psychological evaluations are typically scheduled for 6 hours which allows for for in-depth assessment. For children under the age of 5 and those who have been evaluated within the last year, evaluations may be finished by noon.
What should I expect?
Please arrive on-time to scheduled appointments. Ms. Matvichuk will meet families in the waiting area.
For an evaluation, a clinical interview will generally be conducted at the beginning of the appointment, followed by the testing session. During testing, your child will likely complete a variety of small tasks, similar to what they would do in school. There is no medically invasive procedure and breaks will be provided. Once testing is concluded, a feedback session will be scheduled to review the results and recommendations in detail, either in-person or virtually.
For therapy, Ms. Matvichuk will initially meet with families to gather information, discuss the process of therapy, and establish goals for treatment. Sessions will look different for each individual/family, as they will have different goals and therapeutic approaches for their specific needs.
How should I prepare my child?
You can help prepare your child by ensuring they get a good night’s sleep and eat a nutritious breakfast the day of the evaluation. Explain that they will complete a variety of tasks to help us understand their strengths, challenges, and how their brain works. Emphasize the importance of doing their best. We aim to make the process fun, but some tasks may be challenging.
Should my child take prescribed medications on the date of an evaluation?
Typically we advise for children to take their prescribed medications as usual. However, if you are unsure whether your child should take their medication on the date of the evaluation, please contact the office prior to your appointment.
My appointment is scheduled for 3+ hours. What should we do for lunch?
For full-day appointments, a 1-hour lunch break will be provided. Families are welcome to bring their own lunch or explore nearby restaurants for dining options.
What should I bring to the appointment?
You may bring snacks and beverages for your child. Please make sure your child has their glasses, hearing aids, AAC device, or any other applicable assistive devices. Reports from previous evaluations, school records, and a list of current medications are also helpful. The office is wifi enabled and families are welcome to bring laptops or tablets to use while waiting.

What if I need to cancel or reschedule an appointment?
We kindly ask for at least 24 hours notice if you need to cancel or reschedule your appointment. If an appointment is missed without providing 24 hours notice, a portion of the fee will be charged, except in cases of emergencies.
